Business File Emergency Contact Form - Businesses can fill this form out and send it to the Communications Center to have their business and emergency contact information on file. Business owners can be notified after-hours in the event of an emergency at the business location. This form requests alarm, and emergency contact information. Note: all information is kept completely confidential and used for emergency purposes only.
